Texas Health and Human Services Options
At Holistic Care Home Health Agency, Inc., we are proud to participate in several Texas Health & Human Services (HHS) programs designed to provide personal care and support for individuals with medical needs.
These services help individuals remain independent in their homes by offering assistance with daily living activities, personal care, and household tasks. Our programs include the Community Attendant Services (CAS) Program, Family Care (FC) Services, and the Primary Home Care (PHC) Program.
Community Attendant Services (CAS) Program
The Community Attendant Services (CAS) Program is a non-technical, non-skilled service providing in-home attendant services for people who need assistance with personal care tasks. This program supports adults and children whose health conditions limit their ability to perform Activities of Daily Living (ADLs).
Services Provided:

Escort Services: Accompanying the individual to medical appointments (Note: direct transportation by the attendant is not included).

Home Management: Assistance with household tasks that support health and safety, including:

Cleaning
-
Laundry
-
Grocery shopping
-
Other household chores
-

Personal Care: Help with physical health activities, such as:

Bathing
-
Dressing
-
Grooming
-
Routine hair and skin care
-
Meal preparation and feeding
-
Exercising

Assistance with self-administered medications

Toileting

Transferring and mobility assistance

Who Can Receive CAS Services?
To qualify for the Community Attendant Services (CAS) Program, individuals must meet the following criteria:

A person of any age with functional limitations in performing at least one personal care task due to a medical condition.

Must have a practitioner statement indicating the medical need for attendant services.
-
Must have unmet needs for home management and personal care tasks.
-
Income and resources must not exceed specified limits.
-
Must not be eligible for Medicaid.
Family Care (FC) Services
The Family Care (FC) Program provides non-skilled, non-technical attendant care for eligible adults who are functionally limited in performing daily living activities. These services are designed to help individuals live safely and comfortably at home, without requiring supervision from a Registered Nurse.
Services Provided:

Home Management: Help with tasks that ensure the safety and cleanliness of the home, including:

Laundry

Routine hair and skin care

Preparing meals and feeding

Assistance with self-administered medications

Who Can Receive Family Care Services?
To be eligible for Family Care (FC) Services, individuals must:

Be 18 years or older.

Have an unmet need for home management and personal care tasks.

Have income and resources that do not exceed the specified limits.
-
Meet a specified functional assessment score.
Primary Home Care (PHC) Program
The Primary Home Care (PHC) Program provides in-home attendant services to individuals with an approved medical need for personal care assistance. These services help people who are functionally limited due to their health conditions perform everyday tasks safely in their homes.
Services Provided:

Escort Services: Accompanying individuals to medical appointments for diagnosis or treatment (Note: direct transportation is not provided).

Home Management: Assistance with household tasks that support the individual’s health and safety, including:

Personal Care: Assistance with physical health-related activities, including:

Who Can Receive PHC Services?
To be eligible for Primary Home Care (PHC), individuals must:

Be 21 years or older.

Have an unmet need for home management and personal care tasks.

Have a practitioner’s statement of medical need for personal care services.
-
Be eligible for Medicaid.
-
Meet a specified functional assessment score, demonstrating a functional limitation with at least one personal care task due to their medical condition.
