STAR+PLUS Program
STAR+PLUS is a Medicaid Managed Care Program offered by the Texas Health and Human Services Commission (HHSC). This program is designed to provide long-term care services to adults who are not eligible for Medicare but require assistance with daily living tasks due to disability or chronic illness. STAR+PLUS ensures that qualified individuals can receive comprehensive care at home, helping them maintain their independence and dignity.
What is the STAR+PLUS Program?
The STAR+PLUS Program is a managed care program, meaning the state of Texas contracts with managed care organizations (MCOs) to coordinate and provide health care services. This program offers a range of health care and support services for individuals with Medicaid in Texas, focusing on both medical and non-medical needs.
Through STAR+PLUS, patients receive a coordinated system of care that includes personal assistance, home management, and support services to help them live safely in their homes.
Who is Eligible for PCS?
The STAR+PLUS program is available for:

Individuals without Medicare coverage who require long-term services and supports.

Individuals with Intellectual or Developmental Disabilities (IDD) receiving services in an intermediate care facility or through an IDD waiver.

Individuals with one or more of the following waivers:

Community Living Assistance and Support Services (CLASS)

Deaf Blind and Multiple Disabilities (DBMD)

Home and Community-based Services (HCS)

Texas Home Living (TxHmL)

For children and young adults under 21, enrollment in STAR+PLUS is voluntary. These individuals can choose to participate in STAR+PLUS or continue with traditional Medicaid. Long-term services and supports (LTSS) for children under 21 will continue to be provided through the Department of Aging and Disability Services (DADS).

STAR+PLUS Covered Services
The STAR+PLUS Program provides a range of services aimed at improving the health and well-being of participants.
These services include:
Escort Services
Accompanying individuals on trips to medical appointments for diagnosis or treatment. Note: This service does not include direct transportation by the attendant.
Home Management
Assistance with essential household activities that ensure the patient’s health and safety, such as:

Changing bed linens

Housekeeping

Laundry
-
Grocery shopping and storing purchased items

Washing dishes
Personal Care
Providing help with activities related to physical health and personal hygiene, including:

Bathing and dressing
-
Grooming and routine hair and skin care

Meal preparation and feeding
-
Exercise and mobility support
-
Medication assistance
-
Toileting and transferring/ambulatory assistance
These services are designed to help participants live independently while receiving the care they need in a home environment.

In Texas, Community First Choice (CFC) is available to individuals enrolled in Medicaid, including those receiving services under:

Participants may also choose the Consumer Directed Services (CDS) option for specific CFC services, allowing them to manage their own care services.
